Picture Order Form

Team and individual pictures for each sport are available for order from Steve Mason. Please contact your coach if you have any questions or concerns.

Physical Examination Form

All athletes are required to complete and submit an updated physical form each year. Please contact your coach if you have any questions or concerns.

Medical Release Form

All athletes are required to complete and submit a Medical Release Form each year. Please contact your coach for any questions or concerns.

AISA Participation Permit

All athletes are required to complete and submit a participation permit. Please contact your coach for any questions or concerns you may have.

All athletes are required to complete and submit the Concussion Information Form which acknowledges the risks of harm from a concussion when participating in athletics. Please carefully review and contact your coach if you have any questions or concerns.

Medical History Form

Please complete and submit the Medical History Form for all athletes. If you have any questions or concerns, please contact your coach.

Sportsmanship Agreement

It is mandatory for every Springwood School athlete to complete and submit a signed copy of the Springwood School Sportsmanship Agreement. Students will not be allowed to participate in sports in representation of Springwood School without their Sportsmanship Agreement on file with the Athletic Director.
